It must always be clarified on a country-by-country basis and in connection with the way in which your job title is used, whether there are legal provisions regarding professional secrecy. However, this is usually not the case for hypnosis coaches, hypnotists and/or hypnotherapists.
Nevertheless, it is of great importance and deontologically obligatory.

It concerns all data and information exchanged during the sessions, as well as everything concerning the identity of the accompanied clients.

Some practitioners prefer to stick to video/audio recordings without taking written notes.
